New Patients

Registering is simple, and takes one short visit

Dr. Wong and Dr. Sharma are currently accepting new family practice patients who live in Burnaby and the surrounding area. You do not need a referral.

1

Call or book online

Call us at 604-555-0148 or use the Book Online button to request a meet and greet appointment with Dr. Wong or Dr. Sharma.

2

Complete the intake form

We will email you a short intake form to complete before your visit. Prefer paper? Arrive 15 minutes early and complete it at the clinic instead.

3

Bring your cards

Bring your BC Services Card and one piece of photo ID. If you take regular medications, bring a list of them or photos of the labels.

4

Meet your doctor

Your first visit covers your medical history, your current concerns, and any screening you may be due for. From there, you can book with your own doctor any time.

Moving from another clinic? Ask our reception team for a records release form and we will request your file. Your previous clinic may charge a small fee to copy and send your records.

Policies and Fees

Good to know before your visit

Clear policies help us keep wait times short and appointments available for everyone. If anything below affects you, talk to our front desk and we will do our best to help.

Cancellations and missed appointments

Please give us at least 24 hours notice if you cannot make your appointment, so we can offer the spot to another patient. Late cancellations and missed appointments may be charged a $40 fee, and repeated missed appointments may affect your ability to book ahead.

Prescription renewals

Renewals are normally handled during an appointment, so your doctor can review your medication and check that it is still right for you. Please book before you run out. If your pharmacy faxes us a renewal request, allow two business days, and a $25 fee applies to renewals handled without a visit.

Test results

We will call you if your results need follow up. No news usually means your results were normal, but if you have not heard from us within two weeks of a test, please call the clinic to confirm your results arrived.

One concern at a time

Standard visits are 10 to 15 minutes, which is enough time to cover one concern well. Please raise your most important concern first, and we will gladly book a follow up for anything else.
